Hello and welcome to Becky's World of Wonder.
Please complete all forms prior to your child's first day in attendance. It is very important to have all contact information and medical information. The authorization form must accompany your child on his/her first day in care. Shot records must be on the State cards and the health forms, over the counter medication forms and shot records must be filled out by you and your physician. Please be as specific as possible on all forms.
My hours of operation are from 7:00 am to 5:30pm Monday- Friday
I am not available for evening or weekend care unless it is previously arranged or a "parent's night out." "Parent's night out is a free night of child care I offer a few nights a year as a thank you for choosing me to care for your child(ren).
Payment is due on the Friday prior to the week service is provided. A mandatory late fee of $10.00 per day will be applied, no exceptions.
The client continues to pay for sick and vacation days taken. When a vacation is taken, a POST DATED check for the week(s) they are absent will be given to the provider prior to the vacation period.
The provider will take a one week paid vacation per year. The provider will give as much advance notice as possible. If there is a family emergency the provider will give as much notice as possible and will continue to be paid for said time off but not to exceed a one week period. Each family should find their own back up care for any extended time the provider may need to take off in case of any extended emergency leave. The provider will try to provide a short term back up provider if a short term provider is needed for any appointments. This provider does not take time off unnecessarily and has a very solid attendance record since opening.

Should the client wish to terminate the daycare contract a mandatory full three week paid notice is required whether the child remains in attendance or not.
Should the child cause damage to property in the daycare or steal from the daycare the parent is responsible for replacing or paying for the item damaged or stolen by said child.
A child may not come to the daycare if the child has a fever, is vomiting , or has a contagious disease. A physician must treat all contagious diseases with the proper medication prescribed by that physician for a minimum of a 24 hour period or as otherwise stated by said physician prior to the child returning to daycare. The provider will administer the prescribed medication to the child if the medication is in it's original container with the child's and the doctor's name along with the instructions for dosage is on the label and the medication is no older than six months. The parent will provide any over the counter medication the child may need and it must be on the physician approved "Over the Counter Medications" form filled out by the parent and the Physician and on file with the provider.
The parent will be notified to pick up their child should the child become ill while in care. The child shall be picked up within one hour of notification or as soon as is reasonably possible.
The child will be served snacks and meals while in care depending on the hours they are in care.
Timeout is the form of discipline used in the daycare and will consist of 1 minute for each year of the child's age (if the child is 2 yrs of age the child shall sit in time out for two minutes, if the child is 3 yrs of age the child shall sit in timeout for 3 minutes and so on).
The provider will notify the parent of any injury that occurs while the child is in care. If the injury is severe and require emergency medical treatment, 911 will be called before the parent and then the parent will be called.
If the child is not picked up by 5:30, the time the daycare closes, a $10.00 per quarter hour fee will be assessed. Please be considerate of the hours of operation.
Should the client's check be returned for any reason, a $40.00 returned item fee, plus all fees the provider is charged as a result of that returned check and all late fees incurred by the provider as well as a $10.00 per day fee until the check is made good will be charged. Should a second check be returned by the same client all future payments will be made in cash. Should the client leave the daycare before all fees are paid the provider will seek legal means to collect said fees and will add all legal fees, wages lost attempting to collect said fees, wages paid to a substitute attempting to collect said fees and any legally allowable interest on said fees.
This provider goes beyond the States required safety regulations and has installed a fire extinguisher on each level of the home, a safety chain and a door buzzer on the front and back door and, goes on lock down every time the local school goes on lock down. I also have a fire escape ladder on the second level of the home as the State requires.
We practice monthly fire and safety drills as required.
I am CPR, First Aid, Bloodborne Pathogen and Medication Certified and I complete the required number of continuing education training hours annually. I care about children and take the time to be a licensed provider to be the best I can be.
I will only release your child(ren) to the people specified by you and only after verifying their photo identification. You must call me in advance if there is a change in pickup plans.
Each parent must provide diapers, diaper wipes, disposable gloves, a changing pad that the provider can sanitize after each change and multiple changes of clothes.
When potty training the parents must provide multiple changes of underpants and changes of clothing for the child and must tell the provider what they are doing at home so we can work together on the potty training. This provider will not be able to sit the child on the toilet in intervals of less than 60 minutes because of the other duties that are being performed with all the children in care if the child is not needing the toilet during that time to get the child "used to" sitting on the toilet.
When swimming the parents must provide swim diapers, a swimsuit and towel and sunscreen for their children. The provider will put sunscreen on the children each time the children go outside.
The children are allowed to watch television in daycare however G or PG rated movies and cartoons are the ones watched in the playroom. Movies and cartoons are a "treat" not a daily activity.
I hereby understand that by signing this contract I am required to make payment to Becky Wyzykowski in the amount of $______ per week for my child(ren) _____________________________________________________.
This fee is for no more than 45 hours per week of care. If 45 hours per week are exceeded an additional fee of $_____ per week is required. I understand that payment is due and payable on the Friday prior to the week services are provided and that all fees listed herein will apply as listed. I also understand that I am bound to comply with all of the above rules and regulations.
